9:00 AM in the Cafeteria
Plan on attending if your child will be 5 years old on or before September 30, 2008

Please bring the following:

  • proof of residence/home address
  • certified birth certificate
  • your child's social security number
  • physical examination report
  • current immunization report
  • two emergency contact phone numbers
